New Client Form

Welcome to the Travelers Rest Animal Hospital Family. Clients must be at least 18 years old. We do require all New Clients to pay a deposit fee of $50. This will go towards the exam. Once we schedule an appointment, someone will reach out to you to collect the deposit via phone call. Please fill out and submit the form below.
